Small Pupil Management Algorithm

Managing the small pupil in cataract surgery is not just about enlargement.

It is about anticipation, judgment, and timely intervention.

A pupil that appears “acceptable” at the start of surgery can still become a source of significant intraoperative difficulty if associated risk factors are overlooked. IFIS, intraoperative miosis, dense cataracts, shallow anterior chambers, premium IOL expectations, previous ocular surgery, and systemic medications all influence surgical complexity.

Over time, one principle becomes increasingly clear:

“The threshold for using a pupil expansion device should be low when risk is high.”

Early decision-making often leads to:

• Improved visualization

• Reduced iris trauma

• Lower stress during surgery

• Safer and more predictable outcomes

Whether using iris hooks or a pupil expansion ring, the goal is not merely to “manage” the pupil — it is to create the safest possible surgical environment for both surgeon and patient.

A simple practical algorithm that stays at the back of the mind, will help in surgical planning and intraoperative decision-making.
