The ‘B-OCTA Pupil Expander’ provides an Extra Large (XL) pupil expansion for a wider view during Manual Small Incision Cataract Surgery (MSICS) and
Phacoemulsification (PHACO) in Small Pupil situations as a result of a Non-Dilating Pupil or Intraoperative Miosis.
The B-OCTA is an Octagonal device with notches and flanges. Alternate flanges are tucked under the iris to engage the notches to the pupil margin and
provide pupil expansion. The 8.00 mm device provides a 7.25 mm expanded pupil. The device is further expandable to 8.75 mm by straightening of the
curved flanges during prolapse of the nucleus into the anterior chamber.
The extra-large pupil expansion provided by B-OCTA, its expandable feature and its ability to be deformed into an elliptical shape, facilitate the prolapse of
the largest and hardest nucleus into the anterior chamber during MSICS. The unique design ensures firm engagement of B-OCTA to the pupil margin during
the nucleus prolapse manoeuvres.
Irrespective of the technique used for MSICS, the intact nucleus is prolapsed into the anterior chamber after a large Capsulorhexis or Capsulotomy.
Prolapsing the nucleus into the anterior chamber through a sub optimal pupil, invariably results in further miosis making cortical aspiration and IOL
implantation challenging. Hence, a wide view of the cataract is necessary to perform the steps of Capsulorhexis or Capsulotomy, Cortical aspiration and IOL
implantation safely.
For Phacoemulsification in Supra hard and hard cataracts, some surgeons consider a wider pupil expansion to be safer.
The ‘B-HEX® 23 G Forceps’ allows the surgeon to effortlessly insert the B-OCTA into the anterior chamber and engage it to the pupil margin and also
disengage and it and remove it from the eye.
